- I’ve heard you’ve written an amazing book called, “The Cancer Code.” I haven’t read it yet, but wanted to hear your thoughts on broccolini being an anti-cancer food. I understand it contains Sulforaphane, Indole-3-carbinol (I3C), Glucosinolates, Vitamin C, and fiber. Should I concentrate on cruciferous vegetables, like broccolini, or just work on getting a balanced diet? What are your thoughts? [02:14] I have a little confusion about the difference between taking metformin and injecting insulin. From what I've read in your books it's that insulin is going to make type 2 diabetes worse in the long run. I already know that that’s not the answer to reverse type two diabetes and I’m not taking either one. Like insulin, will metformin also be detrimental to treat T2D or is there a difference between one and the other - and why, please? [05:27] What do they mean by 'immunoreactive insulin'? [12:06] Does the liver respond negatively to the ingestion of supplements? In other words, do the supplements cause any changes in my insulin? [14:52] I started TRE (beginning Jan this year) at about 135 pounds and am now doing 24-hour fasts with OMAD and 2MAD. I have completed a 42-hour fast and will probably try another one this week. I am eating whole foods. Avocados are my miracle!!! I do not snack between meals. My body is still releasing weight with what I am doing now and I feel I have another 5 pounds to go. How will I know when to stop shedding the weight? [18:13] Can you clarify the role of physical activity and exercise in weight loss? In The Obesity Code, you shared that a number of rigorous clinical trials demonstrate that exercise contributes minimally, if at all, to weight loss. However, zone 2 exercise is promoted for its'fat burnin' benefits. I plan to continue exercising for its health-promoting benefits but I would really like to hear your take on the extent to which it can assist with weight loss, and what exercise types you recommend for weight loss. [21:56] On non-fasting days, what is Dr. Fung’s chosen style of eating? [29:46]

- I’ve known for a long time how neuroprotective ketones are for the brain, but, wow, what ketones do for the heart is amazing, especially the failing heart. Could you elaborate, Dr. Fung? [02:15] Do you need to be in ketosis to burn fat and does being in at least a low level of nutritional ketosis help you go into autophagy sooner? [04:39] Why does it take such a long time to become fat adapted? [07:20] I’ve heard that fasting “turns on” survival genes. Can you tell us a little more about this? [09:25] What are your thoughts about the new recommendation to carb load before surgery to “calm” someone? [11:12] Is there any correlation between fasting and pulse rate? [12:58] I have a diabetic friend who is participating in a study on duodenal mucosa resurfacing (DMR) as a treatment for T2D. Does Dr. Fung have any take on this pretty new procedure or explanation of how and why it works? [13:17] If a person wants the benefit of therapeutic fasting (like autophagy) but they do not have adequate body fat for longer fasts (for example, a TOFI person with a fatty liver, or somebody at 30% body fat who needs a lot of autophagy for an autoimmune condition), How should they eat on eating days to be able to maintain a therapeutic fasting schedule like an alternate-day fasting protocol? [16:33] What are the signs of a histamine intolerance and can it get worse over time? Does fasting help? [17:53] If autophagy is mostly switched on by the presence of mTor, could a person (in principle) eat a zero protein diet and eat exclusively fat but still ramp up autophagy enough for some protein recycling to aid loose skin reduction, macular problems, skin tags, etc.? [18:45] How do you get rid of visceral fat? [20:22] How long does the effect of dairy on insulin last? [21:45] Why is it easier for women to do longer fasts after they’ve had a small amount of tubers during the last meal before their fast? [24:41]

1. Which fasting protocol is best for getting rid of excess skin while you also get rid of excess weight? [01:58]
2. My GP has told me to take medication three times a day. How do I incorporate intermittent fasting into my lifestyle when I have to take meds three times a day? [09:26]
3. I’ve been fasting for two years. I lost 20 pounds the first year but gained 10 back in the second. I try to do 42-hour fasts but end up breaking it around the 20-hour mark. My blood glucose numbers are up and my weight isn’t coming down. I am a 45-year-old female. I am ready to give up. Please help. [13:50]
4. I was recently diagnosed with vulvar lichen sclerosus (LS). My gynaecologist says that it is believed to be an autoimmune disorder affecting the skin of the vulva. It is a horrifying condition that can cause changes to the architecture of the vulva, intense itching, tearing of the skin, etc. I’ve never known anyone to have this condition, but my gynaecologist says that she has a lot of patients with LS. Have any of your clients seen improvement in their LS? Is there a particular fasting protocol that you would recommend. I’m just devastated by this diagnosis. [21:43]
5. I understand that snacking (even with healthy foods) is a bad idea because it keeps your insulin levels up so how many meals would you suggest is the most effective during a typical 8-hour window in a 16/8 IF regime? Or maybe a better way to look at it is how many hours should I leave between meals to allow insulin to go back down? [26:55]

1. I know you have mentioned chia seeds (in water) as a way to gently break your fast, but can I use chia seeds as a training wheel during a fast because they give a feeling of fullness? [07:25]
2. I do 18:6 daily and a 48 each week. It took much effort to get my gut health stabilised. I have completed a couple of 5-day fasts in the past, but now I am worried about what a 5-day fast does to the good guys in my gut. With no fibre coming in, how do they survive? I don’t want to compromise my good gut flora, but I’d like the benefits of autophagy and stem cell generation. [13:44]
3. I’ve been ADF’ing for 6 weeks. I typically fast 36hrs, only consuming plain water, salt, and 2-3 cups of black coffee. I am 46 and have had seven pregnancies. I have hardly lost any weight, only a couple pounds. What am I doing wrong? On my eating days, I eat 3 meals over 12hrs, lower carb but not keto, and add 2 Ts cream to my coffee those mornings. I mostly do not snack. I find it very hard to fast 42hrs. Would three 42-hr fasts be more effective than ADF with 36hr fasts? Does it take a while for results when peri-menopausal? [18:42]
4. As someone “recovering” from PTSD, I have been taught skills and tricks to reduce cortisol and adrenaline. From a recent podcast I learnt that fasting increases these and I am now concerned that it may create effects counter to what I am trying to achieve to reduce PTSD symptoms. Have you had patients with a similar concern? Is there perhaps a difference in duration, or other helpful hormones released at the same time, to differentiate it from the perpetually elevated stress hormones after PTSD? [28:21]

1. I’ve heard Dr. Fung say to skip breakfast, but recently in one of Dr. Nadia’s groups, I heard her recommend early time-restricted eating. Could you clarify this for me? [00:47]
2. This coming February I will have been fasting for two years. I fast 18 hours a day and exercise almost daily. I have hit the world's biggest plateau the last 6 months and just cannot lose any more inches/fat/weight. I'm definitely becoming more toned, but I'm not really getting smaller. I'm wondering if I'm at my body's resting weight or if I need to shake things up with my eating? I eat very balanced meals with protein, carbs and veggies. I could probably stand to NOT eat a cookie here or there. Since listening to your podcast, I have been sprinkling in one 24-hour fast a week and this week I am aiming for two 24-hour fasts. [08:26]
3. I've been fasting for a couple of months. I have read a lot about the fasting cycle with the menstrual cycle and I have heard you say it's easier to do longer fasts in the first week of the menstrual cycle. That's actually what I'm doing - three 36 or 42-hour fasts in my first week. The rest of my cycle I do 18-hour fasts and only 12-hour fasts in my 4th week. With that «protocol» I have lost 20 pounds in 3 months, but should I limit those longer fast to the first week of my cycle? [14:13]
4. How much does gluconeogenesis/the dawn phenomenon impact blood sugars early in the morning? A family member who is fasting and is type 2 diabetic has blood sugars of 150 or so at 6am but 95 later in the morning during a fast. I suspect the dawn phenomenon but I have no idea how much it could impact someone’s blood sugars. [20:20]

1. I have IBS-C and a gluten sensitivity, along with ever changing hormones as I am perimenopausal. I’m trying to find the best regimen to help. Of course weight loss would be nice but my main goal is to rid myself of abdominal pains, severe constipation, and nausea. [01:43]
2. My mom has type 2 diabetes and is taking Metformin. I have convinced her to do at least 16 hours of fasting a day and she has been doing this for 5 months or so. She says her glucose levels are much more stable but her cholesterol has gone up. Should she worry? [09:21]
3. Once you have reached your fat-loss goal, how do you transition to a maintenance schedule without gaining back the weight? And what would a good fat-loss maintenance schedule be? [15:10]
4. I started fasting about a year and a half ago with a very mixed schedule and was successful in losing about 25 lbs in 8 months. Since then I’ve been at a plateau for another 8 months. I find it hard to not eat at least once per day. What are your thoughts on ADF with the 500 cal vs without the 500 cals? [17:35]
5. I don’t think I’ve heard you discuss the sweetener erythritol. I realize that it may still not be great for sugar cravings, but does it affect insulin? If I had a small amount in my tea, would it break my fast? I’ve seen conflicting messages? [25:16]

3. Do you have any tips for fasting on a vegetarian diet? My partner is a vegetarian and I am interested in cutting out meat also, but I am not sure about the nutritional value of meat substitutes like tofu and other plant-based proteins. [07:28]
4. I would love to hear more about fasting with a plant-based diet. I don't avoid fruit or below-ground veggies. I eat nuts and seeds, but no dairy. I have cut out most highly processed foods and I am doing TRE, usually a 16-18 hour fast. There is so much conflicting information about diet! [09:57]
5. I follow a high-fat, low-carb diet on my eating days. I seem to struggle to keep my carbs low while trying to get in my daily fiber. I try to eat as much high-fiber food as possible (like avocados, chia seeds, and vegetables) at each meal. How do you add fiber to your diet while staying low-carb? 6. Being that fiber helps to slow the digestion of sugars and fats (therefore decreasing blood glucose spikes), would you recommend taking a daily fiber supplement? What about when fasting? Does it technically break a fast even though the body is not digesting it? [18:15]

2. I am planning on doing 24-hour fasts three days a week for fat loss. For this schedule, you typically recommend (on eating days) to still fast 16 hours and only eat two meals. But, on my eating days, would it be better to not fast at all and eat three meals to keep up my metabolism? If not, how do I maintain my metabolism during this process? [08:08]
3. I am doing three 42-hour fasts a week. On my fasting days, every time I reach 24 hours into my fast, I start feeling cold. What is the process behind that reaction?
I’ve also found that since I started fasting eight weeks ago, my perimenopause symptoms have increased, namely I experience more and more severe night sweats. Is this just a coincidence or could fasting be increasing these symptoms. [14:06]
4. I started my fasting journey after listening to your podcast, which I love. I'm now up to 24 hours three times a week and looking forward to doing longer ones soon!
My question for you is about gestational diabetes. I’ve had it with both of my kids and was told that eating healthy has nothing to do with getting it. I’m curious what you think about this. I’m determined to get healthy through fasting to prevent T2D in the future and am hoping to prevent gestational diabetes as well as pre eclampsia (I had hypertension with my first and pre eclampsia with my second!) Is there a way to prepare my body to prevent either of these with my next pregnancy. [20:41]
5. Do pregnant women have to eat more than three good meals a day? I get full so quickly and I’m not sure I can eat more than three meals a day. I normally ate two meals a day when I fasted before pregnancy. [25:05]

2. I’ve always had a lot of charley horse type of leg cramps and abdominal muscle cramps. usually these happen more when I’m doing a low-carb diet. However, when I’m fasting these muscle cramps happen far more frequently. The cramps seem to improve and not be as frequent if I am well hydrated but they get worse if I drink a lot of coffee and also of I drink wine on my eating days. Should I be adding more salt perhaps? 3. I get restless legs (and arms) most nights when I am trying to get to sleep. Someone suggested taking magnesium. Might this help? [10:40]
4. I get really bad breath when I am fasting. I’m now a bit paranoid about kissing my partner. Help! [21:23]
5. I have rosacea that is managed by diet (not eating certain things) and careful skin care. When I hit 24-26 hours into my fasts, my rosacea goes crazy! My face is red hot, and my skin is bumpy - it’s as bad as if I had eaten hot peppers. It usually lasts 2-3 hours and then goes away. My esthetician says maybe it was an allergy to the green tea I drink in the evenings, but I switched out teas and even cut it out and it still happened. Fasting is the only consistent thing that causes this flare up and it can’t be a coincidence. Any idea why this happens? [28:01]
6. I have terrible body order coming from my armpits when I fast. I've had to buy special odor-eliminating laundry detergent to get rid of the smell from my clothes. Do you think its just part of fasting and losing weight? I've heard of fat holding estrogen. Is it releasing that IVF estrogen finally (approx 5 years previous) or is it just a perimenopausal changing hormones thing? I don't think the latter because it began when I started ADF again and I don't smell on eating days. [23:58]

1. I have two preschool aged children and I really want to set them up for life with a healthy relationship with food. Obviously, I don’t think we should fast children but I have noticed that their lives revolve around their mealtimes and snacks. Is TRE something that would benefit them? At what age should we implement this? [03:21]
2. I have 12 and 16-year-old girls. There are so many body image influences out there. I am concerned if they see me doing extended fast that they will not understand why I am doing it or that they might develop an eating disorder. [13:16]
3. My daughter is 13 years old, tall, slender and active/a multi-sport athlete with no weight-related concerns. She started her period about 6 months ago and luckily it’s been pretty regular and she’s managing it very well. The only hormonal issue I’m noticing so far is that she’s struggling pretty badly with acne on her face and upper back. I just want her to be happy and healthy and to fuel her body appropriately, especially during all these hormonal changes her body is going through. [22:42]
4. My wife says she can't fast with me because she had weight loss surgery (about two years ago). She would like to do low carb/keto-ish/fasting with me but says she can't eat enough in a meal to last her 5 or 6 hours. Is there a protocol for fasting with a smaller stomach or it is not recommended? [30:07]
5. When I fast for 36 hours (which up to now has been my longest fasting period) I gain five pounds during the day when I start eating again. I feel bloated and very unwell. I suspect it is water retention as it goes away on the next day. Is this normal? [35:20]
6. My question relates to a previous episode where Nadia mentioned that the smell of food can cause an insulin response. As someone who has insulin resistance and happens to be a professional chef, that news was alarming! Is this actually the case, or did I hear that wrong? [38:34]
