Dr. Martindale describes his fascinating career path. While working in a lab at UCLA during the summers to earn money for forestry school, Dr. Martindale became interested in human physiology. After completing his undergraduate studies, he was invited back to UCLA to work in the nutrition department as a graduate student. Here, he earned his PhD in Nutrition. This interest in nutrition then led him to enter medical school, specializing in surgery, all while enrolled in the Army which allowed him to pay for medical school. This video features Dr. Nisha Mangalat, Associate Professor of Pediatrics in the Division of Gastroenterology at the St. Louis University School of Medicine and Medical Director for the Glennon Intestinal Rehabilitation and Feeding (GIRAF) Program at SSM Health Cardinal Glennon Children’s Hospital. The GIRAF program was formed around a multidisciplinary, collaborative approach to caring for complex pediatric patients with intestinal failure. The GIRAF team meets weekly to discuss patient cases and review updates provided by the patients’ parents/caregivers. This enables the team to make proactive, systematic adjustments to each patient’s nutrition therapy. Dr. Mangalat shares that the name “GIRAF” was coined by their registered dietitian while attending an ASPEN meeting. Dr. Mangalat holds nutrition education very close to her heart and notes several gaps in the current medical education curriculums. Dr. Mangalat is the director of the pediatric gastrointestinal fellowship program and offers some insight into how we can engage future physicians to get involved in nutrition. This video features Dr. Ruba Abdelhadi, Professor of Pediatrics at the University of Missouri-Kansas City School of Medicine and Director of Nutrition Support at Children’s Mercy Hospital. She serves on the ASPEN Board of Directors and also serves on the National Board of Physician Nutrition Specialists. Dr. Abdelhadi developed an interest in nutrition when she was asked to step in and develop a one-week nutrition curriculum for medical students. She was surprised to learn that there is no dedicated nutrition content in undergraduate and medical education. Dr. Abdelhadi shared two priorities. The first is to identify malnutrition in every clinical encounter and not miss out on opportunities to address it. The other is to provide nutrition education to all medical students, regardless of their area of specialty or education level. She emphasizes the importance of collaboration, noting that multidisciplinary work offers so much to not only patients but also professionals. Dr. Abdelhadi recommends that young professionals identify their passion and then find opportunities in all scholarly areas—teaching, clinical work, and service. Alverdy, Sara and Harold Lincoln Thompson Professor of Surgery and Executive Vice Chair of the Department of Surgery at the University of Chicago Medicine in Chicago, Illinois. This video features Dr. Justine M. Turner, MD, PhD, Professor of Pediatrics and Divisional Director of Gastroenterology and Nutrition at the University of Alberta in Canada. Dr. Turner’s interest in nutrition stemmed from her original interest in Gastroenterology. Looking back, she felt there was a lack of nutrition education in her medical training, which she only realized once she began to pursue her PhD in nutrition. She notes there are too few physicians with PhDs in nutrition. Dr. Turner spends 40% of her time on research and 35% on clinical practice. Currently, she runs a translational animal lab looking at growth factors for adaptation, TPN lipids, and enteral nutrition in short bowel syndrome. In her clinical practice, Dr. Turner deals mostly with Celiac disease and gluten free diet while her inpatient service looks after solid organ and intestinal transplant patients. She highlights that nutrition is integrated in everything that she does. She also shares her insights on the differences between the delivery of healthcare in Australia and Canada and how each were affected during the Covid-19 pandemic. She insists that there is no better home for her interests than ASPEN. It has provided her with incredible opportunities to collaborate, present her data, and receive recognition for her work. ASPEN is also a reliable place for her students to learn and grow. She has served on the ASPEN research committee and currently serves as the Chair of the Publication Education Committee. Dr. Turner has three pieces of advice for junior faculty: find multiple mentors, be adaptable, and to say “yes” to be on committees or societies. This video honors Dr. Praveen S. Goday, MBBS, Director of Feeding and Nutrition Programs at Nationwide Children’s Hospital in Columbus, Ohio. Dr. Christina Valentine discusses her NIH funded research on DHA supplementation for lactating mothers of premature infants. They have shown that 1000 mg of DHA daily taken by the mother decreases preterm infant inflammation through human breast milk ingestion. This video honors Dr. Mark Corkins, Division Chief of Pediatric Gastroenterology at the University of Tennessee Health Science Center and Le Bonheur Children’s Hospital. Dr. Corkins said he was fortunate in that nutrition was a focus in every step of his training. In medical school, his biochemistry professor said that students didn’t get enough nutrition and was given an hour each week to teach it. He later found out this focus on nutrition was not the norm in medical training. Dr. Corkins works as an academic physician and loves to teach nutrition. This video honors Dr. Maitreyi Raman, Associate Professor of Medicine at the Foothills Medical Centre in Calgary, Alberta in Canada. She is also the Medical Director of the Southern Alberta Home Enteral and Parenteral Nutrition Program. This video honors Jose Pimiento, MD, FACS, Associate Professor of Surgery, in the division of Gastrointestinal Oncology, at the Moffitt Cancer Center in Tampa, FL. Dr. Pimiento had the honor of training with Dr. Stanley Dudrick, the father of intravenous feeding. He shares some of Dr. Dudrick’s philosophy that as a surgeon, you need to be capable of handling everything, including the nutrition of your patients. Dr. Dudrick was upset with the approach of waiting for the next day to feed patients— tomorrow the patient will not have tests and will eat better, and then tomorrow comes and you’re doing something else on the patient, and so on. He was surprised that every day, we would replace electrolytes, but never think about replacing calories and protein. Dr Pimiento also talks about his innovative research work in nutraceuticals and cancer chemoprevention. He ends by sharing some advice for those starting out—finding interdisciplinary, local, and global networks of people with similar interests. In November 2020, the Durable Medical Equipment Medicare Advisory contractors retired the Local Coverage Determinants (LCDs) for home parenteral and enteral nutrition (PEN). Lipid injectable emulsions (ILEs) are complex pharmaceutical formulations intended as a source of energy and fatty acids for parenteral nutrition (PN). Part 1 of this series addressed adult use. Part 2 addresses ILE safety in neonatal and pediatric patients, who often require higher doses compared with adults to support growth, development, and daily metabolic needs. ILE is often administered as a separate infusion owing to compatibility and stability issues and limitations of access. It is associated with PN errors in the administration, prescribing, and transcribing processes and has been associated with adverse effects, including effects from doses exceeding manufacturer recommendations. This paper describes safe use of ILE products for US neonatal and pediatric patients, including indications, prescribing, order review, preparation, administration, and monitoring. This Spotlight honors David S. Seres, MD, ScM, PNS, FASPEN, director of the Medical Nutrition Program and associate professor of medicine in the Institute of Human Nutrition at Columbia University Medical Center in New York City. He has over 30 years of experience in nutrition support consulting, clinical research, and teaching. He’s currently the section editor for nutrition at UpToDate. Dr. Seres shares his thoughts on how the field of nutrition has changed, and not changed (lack of randomized control trial data), and what’s to come (personalized nutrition). He leads one of the few nutrition fellowship programs and points out that in the ICU, patients on average receive only 45% of the calories prescribed to them. Dr. Isabel Correia discusses her paper, "A Nutrition Therapy Cost-Effectiveness Model Indicating How Nutrition May Contribute to the Efficiency and Financial Sustainability of the Health Systems." Malnutrition is highly prevalent in hospitalized patients but seldom recognized and treated. Malnutrition poses several adverse events, such as increased infection rates, length of hospital stay, and mortality, as well as costs. Early nutrition interventions have been shown to decrease the associated malnutrition burdens, leading to relevant savings. This podcast discusses two adult patient cases with mixed oil ILE refractory Intestinal Failure Associated Liver Disease (IFALD). IFALD which has been associated with soy-based ILE, has seen some improvement with the use of mixed oil ILEs. This Spotlight honors Shirley Paski, MD, MS, FRCP(C), director of small bowel diseases and nutrition at Cedars-Sinai Medical Center in Los Angeles, CA. Dr. Paski, who completed her residency in Canada, shares some of the differences between Canadian and US healthcare. This Spotlight honors ASPEN’s 42 president, Nilesh Mehta, MD, FASPEN, anesthesia professor and critical care nutrition & metabolism chair at Harvard Medical School in Boston, MA. During his early years of practice, Dr. Mehta realized there was very little evidence-based guidelines to drive nutrient delivery to sick children at the bedside. Depending on the team, the child was fed differently and the fundamental question of how much food/calories to deliver was unanswered. This began his journey into research.
