The Patient Safety Incident Response Framework (PSIRF) is a new approach to learning from patient safety events in a way that leads to improvement in care. It was published in August 2022 and mandated in the NHS Standard Contract in April 2024. In this podcast, GPs and medical examiners from England and Wales discuss their experience of extending medical examiner scrutiny to deaths in community settings. It explains how partnership working between GPS and medical examiners improves the experience of bereaved people and explores advantages for GPs of starting work with medical examiners now, before medical examiners become a required part of the new statutory death certification process from April 2024. Medical examiners are senior doctors from all specialties including GPs, who provide independent scrutiny of deaths not taken for investigation by coroners, and offer the bereaved an opportunity to ask questions and raise concerns. A key benefit of the new Learn from Patient Safety Events (LFPSE) service is its introduction of machine learning to hugely enhance the NHS’s capabilities for processing and analysing records of patient safety events. The digital workforce team at NHS England is producing a plan to narrow the skills gap for digital, data, technology and informatics roles. In this podcast, senior digital leaders come together to discuss what they think is fundamental to the success of the plan. Key to the success of PSIRF is the application of system-based approaches to learning from patient safety incidents. This podcast focuses on the training NHS organisations should ensure staff undertake as part of their preparation for the implementation of the Patient Safety Incident Response Framework (PSIRF) by Autumn 2023. The podcast provides an introduction to the training requirements, the PSIRF training procurement framework, training offered by the Healthcare Safety Investigation Branch, and an overview of how this fits in with the wider national patient safety syllabus. In this podcast, you can learn more about what autism is, what it’s like to be autistic and how to make simple, reasonable adjustments. It features a discussion with Dr Anita Pearson, GP and National Speciality Advisor, Autism and two team members with lived experience: Aaron Senior who is autistic and Fazilla Amide, a family carer, who are all employed within the Learning Disability and Autism programme at NHS England and NHS Improvement Dr Pearson, Aaron and Fazilla provide some helpful ideas in terms of how to support autistic people and their families, provide some key facts around autism and some of the considerations around health inequalities and how you can make appointments go even more smoothly. They talk about how making reasonable adjustments for autistic people can be simple to do and can make a real difference to people’s health outcomes.

Podcast from the NHS England and NHS Improvement National Patient Safety Team, where Tracey Herlihey, head of patient safety incident response policy, and Lauren Mosley, head of patient safety implementation, talk about the Patient Safety Incident Response Framework (PSIRF) which will be launched in Spring 2022. In a wide-ranging conversation Sean Duggan, Chief Executive of the Mental Health Network at NHS Confederation, and Melanie Walker, Chief Executive of Devon Partnership NHS Trust, take a detailed look at provider collaboratives and the fundamental role they play in integrated care systems. With a particular focus on mental health, they draw attention to the need for early intervention and investment to strengthen services to ensure that there is parity of esteem with physical health. The main opportunity they cite is that if you’re getting mental health provision right, then you’re in a much better position to alleviate pressures in other areas of your ICS.

James Pollitt, Assistant Director of Strategic Development at Sandwell and West Birmingham NHS Trust, and Jean Templeton, Chief Executive at the youth homeless charity St Basils, discuss how the NHS in their ICS is working with partners to enable social and economic development, and why other systems should be following suit. The lively discussion sees James and Jean discuss how partnership working across the system is tackling health inequalities head on, utilising empty NHS buildings more effectively, and how the local economy has been strengthened for the benefit of the whole community. Autumn’s Integrated Care Podcast Series kicks off this week with a deep-dive into Somerset ICS’s journey to substantially improve mental health services through NHS, local authority and VCSE partnership. Their way of working, “a masterclass in positive risk taking and leadership” as Jane Yeandle from Somerset NHS Foundation Trust puts it, has seen a model influenced by co-production and an innovative approach to commissioning. It’s all guided by people with lived experience of navigating complex systems. The ‘no wrong door’ approach has maximised the mental health support available to individuals while creating a significant feelgood factor across the system.

GP Dr Ursula Montgomery is joined by secondary care clinicians, Professor Joseph Dias and Professor Chris Moran, to discuss how closer working between primary and secondary care has benefited patients waiting for the next stage of their care. Find out how new and innovative ways of working during the pandemic have become the norm, including the use of system wide resources and greater patient outreach with targeted help and support to ensure there is a focus on quality of life for the patients as they wait.

A discussion on how partnership working is building stronger ties between housing and health that is seeing positive outcomes for local populations. Work that is achieving vast efficiencies, while also seeing a reduction in people needing to contact their local GP or NHS111. All accomplished in part by focusing on the solution rather than the process, taking the time to get it right and remaining resilient when faced with resistance.

Three Partners in General Practice come together to discuss their journey towards becoming a Partner in their practice, the support available to them through schemes like the New to Partnerships Payment Scheme, and why they would recommend others consider taking up a partnership opportunity. NHS England and NHS Improvement, Learning Disability and Autism, National Clinical Director is in conversation with Fazilla Amide, whose 17 year old son has complex health needs, learning disability and autism. Frontline clinicians discuss how collaborative working has ensured care homes and local health services have been able to come together as part of primary care networks to successfully deliver a challenging vaccination programme, while also providing enhanced healthcare for patients and greater support for staff. We also hear how integrated care systems have been able to strengthen this work offering strategies that, among other things, have ensured a redistribution of resources to where they’re needed most during the pandemic.

The creation of a shared purpose is seeing great improvements for patients in the Sussex Health and Care Partnership, as organisations in the integrated care system work closer together to meet the changing needs of the local population. A great example of this is the Mental Health Collaborative that recognises mental health and wellbeing as the responsibility of the whole health and care system. And, in this 17 minute podcast we hear from those involved in the collaborative and how closer working and gathered learning that informs future practice, centred around the patient’s perspective, can reap huge benefits for residents.

With Population Health Management (PHM) charting the way for systems and primary care networks (PCNs) to precisely understand the people in their communities who need more support we hear from a team in Berkshire West integrated care system (ICS) already using these techniques. Bringing together colleagues from across the system – GPs, data analysts, acute specialists, third sector and many more – they started by working together more effectively then analysed data using new techniques and finally implemented local solutions to address individual patients’ needs. Their results have shown how this integrated way of working improves patient outcomes, reduces pressure on professionals and saves money. With prime examples of why PHM is a critical building block for ICSs and PCNs alike, this discussion will aid anyone wanting to efficiently deliver true personalised care and better health for decades to come.

Michael Wood, Head of Health Economic Partnerships at NHS Confederation leads a discussion with colleagues in Dorset integrated care system (ICS) about how place-based partnership working, particularly between the ICSs, Local Enterprise Partnerships (LEP), universities and others can be of enormous benefit to a system and its population. It's the forging of mutually beneficial partnerships to influence cross-sector plans that has seen Dorset able to have a more joined-up approach to workforce challenges and service provision.
