In this show, Nurse Mo breaks down a wide range of nursing school concepts so they’re easy to understand. Each episode aims to help nursing students improve their study techniques, pass nursing school exams, and feel more confident in clinical. If you’re nervous about nursing school, not “getting it” in lecture, or feeling overwhelmed with all the information you have to learn, this podcast will help you optimize your study time so you can learn more with less stress. While nursing school is challenging, it doesn’t have to be impossible. When concepts are explained in a way that “just makes sense”, nursing students have better recall on exams, feel more confident working with patients in clinical, and are better prepared for NCLEX. Nursing school exam questions place you into a clinical scenario where you have to make decisions based on the information provided. Many times these scenarios are robust and full of a lot of details such as vital signs, past medical history, and the patient’s signs and symptoms. When you have a complex scenario such as this, it is called a case study. Case studies can be challenging for students for a variety of reasons: They can be lengthy They contain a lot of information, much of it numerical data They often contain distractors or irrelevant information They may contain abbreviations or terms you don’t understand You might not be sure what to do with all the information provided The key to conquering case studies is to use a systematic approach that you follow every single time. Local anesthetics are medications that block nerve impulses so that pain signals aren’t transmitted to the brain. The result is that the anesthetized area is numb and pain is controlled. Just because local anesthetics are “local” doesn’t mean systemic toxicity can’t occur. Local anesthetic systemic toxicity (LAST) occurs when local anesthetics extend into the system at toxic levels. It most typically occurs due to the physician accidentally injecting the medication into the intravascular space. However, it can also be from locally administered overdose, which leads to systemic toxicity. Thu, 07 Mar 2024 - 391 - #335: Your Pacemaker Questions Answered
When the heart rate is irregular or too slow, cardiac output is compromised. A pacemaker may be utilized to support the heart’s electrical system, stabilize arrhythmias, and ensure the heart pumps at a rate that supports tissue perfusion throughout the body. A pacemaker may be indicated in patients with arrhythmias such as sick sinus syndrome (also known as sinus node dysfunction), symptomatic bradycardia, or high-grade heart blocks. Other indications include post myocardial infarction, heart failure, congenital heart defects, and temporarily after some cardiac surgeries. This episode answers some key questions you have about pacemakers including: What do all these terms mean? Capture? Sensing? Output? What’s the difference between failure to sense and oversensing? RSI is rapid sequence intubation, a procedure performed to emergently intubate a patient with an intact gag reflex in cases of acute respiratory failure and/or cardiovascular collapse. As a nurse working in the emergency department or ICU, you will be obtaining and may be administering the medications used to perform a rapid sequence intubation. Please be aware of your scope of practice and facility protocol regarding the administration of these medications. In some cases, the medications can only be administered by a physician. Which combination of medications the physician chooses will vary depending on a variety of factors including the patient’s condition and physician preference. A common combination is a sedative with a short-acting paralytic, though analgesics may also be utilized. Epidermal necrolysis refers to severe cutaneous reactions that cause extensive necrosis of tissue and detachment of the epidermis. Being able to decipher medical language means understanding the common roots, suffixes and prefixes. For example, if you know “ectomy” means the removal of something…even without recognizing the rest of the word, you at least know the patient had some kind of surgical procedure and something was removed. Orthostatic hypotension (OH) occurs when the body has an inadequate response to postural changes and, as a result, is unable to maintain a steady blood pressure when moving from a lying to standing position. It is defined as a decrease in systolic or diastolic blood pressure that occurs within three minutes of moving from a sitting or supine position to a standing position. The parameter for OH is a decrease of 20 mmHg systolic or a decrease of 10 mmHg diastolic. In nursing school, I learned that digital clubbing was a key sign of COPD and I accepted this as gospel and moved on with my life. I used this information to answer exam questions on the topic and I always included assessing for it in my care plans for patients with COPD. But, like all things in nursing, there’s so much more to this story. As the nurse, one of your key responsibilities is to notice changes in patient condition…and that can be difficult to do in a timely manner if you’re not assessing your patient on a very consistent basis. I call this the “doorway assessment” because much of it can be done from the doorway! I usually saw this complex condition in the MICU due to patients who were “found down” and had been lying in one position, completely immobile, for extended periods of time.
